Smoking and Your Eyes: The Hidden Risks

Medically reviewed by Dr. Jayadatt D. Patel — MBBS, MS (Ophthalmology), FCRS · Last reviewed June 2026

Smoking is well known to harm the heart and lungs — but it also damages the eyes. People who smoke face a meaningfully higher risk of several sight-threatening conditions, and quitting reduces that risk.

How smoking harms the eyes

  • Age-related macular degeneration (AMD) — smoking is one of the strongest modifiable risk factors
  • Cataract — tends to develop earlier in smokers
  • Diabetic eye disease — worsened by smoking
  • Dry eye and optic-nerve problems are also linked

Why it matters

Tobacco smoke promotes inflammation and oxidative stress and reduces blood supply to delicate eye tissues — accelerating damage to the retina and lens.

The good news

Stopping smoking lowers the risk of these conditions over time, at any age. Combined with regular eye checks, it is one of the simplest things you can do to protect your sight.

Frequently asked questions

Does quitting smoking actually help my eyes?

Yes — risk of conditions such as AMD falls over time after quitting, and other eye-health benefits follow.

Is passive smoking bad for the eyes too?

Second-hand smoke can irritate the eyes and is best avoided, particularly for children and people with existing eye disease.
