Fuchs’ Dystrophy: When a Corneal Transplant Helps
Fuchs’ endothelial dystrophy is a common reason for corneal clouding, especially later in life. It develops slowly as the cornea’s inner “pump” layer (the endothelium) gradually fails — and modern surgery can restore clear vision with a small, targeted transplant.
What happens in Fuchs’ dystrophy
The endothelium keeps the cornea clear by pumping out fluid. As its cells decline, fluid builds up and the cornea swells — causing misty, glare-prone vision that is often worse in the mornings and clears a little through the day.
Early management
Early on, symptoms may be eased with simple measures while vision is monitored. Cataract surgery, if needed, is planned carefully because it can stress the endothelium.
When transplant helps
When vision is significantly affected, an endothelial transplant (DMEK or DSEK) replaces only the failing inner layer through a small incision — restoring clarity with faster recovery than a full-thickness transplant.
